Maryland Children’s Health Program (MCHP/Free and Premium Programs)
You can apply on a walk-in basis at the Frederick County Health Department at 350 Montevue Lane during the following times:

At the Frederick County Health Department, you will meet one-on-one with one of our Caseworkers. The Caseworker will create an online account for you through the Maryland Health Connection website and assist you with completing the new or renewal application. If you need verifications, the Caseworker will help you complete that process as well. Once an MCO (Managed Care Organization) and PCP (Primary Care Doctor) are selected, the Caseworker will complete the entire enrollment process with you here.
To be eligible for Maryland Medical Assistance programs, individuals and families must meet annual income requirements. YOU ARE ELIGIBLE if your income DOES NOT EXCEED the figures below according to the number of people in your family.

Premium cost is per family/household each month.
If your income exceeds the requirements shown above, you may be eligible for a Qualified Health Plan and referred to the in-house Navigator after your eligibility decision has been made.
